The West Yorkshire RECONNECT team is built up of multi-disciplinary staff members with a variety of specialisms.

RECONNECT Project Support Officer
RECONNECT Navigators

Engage with individuals prior to release from prison and following release, support them to transition back into the community and engage with local health and other community-based services.

Mental Health Practitioners

Undertake intensive mental health and social care assessments to identify those with health vulnerabilities and need of support services on release ensuring detailed care plans are in place.

Peer Support Workers

Bring lived experience of the criminal justice system to support individuals who may present with a range of vulnerabilities that may be linked to their offending behaviour.

Our lived experience team support complex clients that can be hard to reach and a challenge to engage by using their experiences of the criminal justice system and other services. The invaluable dynamic that Peer Support Workers have enhances the service and provides the opportunity for colleagues to better understand the lived experience of our service users.
